Outreach Testing Team

Thanks so much for your interest in joining the Outreach Testing Team! These specialized events operate in a number of different environments to make testing as accessible as possible to all community members.

Please review the information below in full before signing up for outreach testing.

When using the sign-up page below, please enter your full name, email address, and phone number after selecting a volunteer spot. After you sign up for your first outreach testing spot, one of our volunteer coordinators will reach out to you by phone to discuss some additional details.

Our times for testing at many locations is quite limited. All volunteer positions for this program are incredibly important to ensure a smooth operation! If you can't make it to a spot you selected, please contact us as soon as possible so we can try to arrange coverage.

Finally, please note that everyone on-site for outreach testing must wear closed-toe shoes and long pants.

Volunteer Screening & Waiver

Volunteers must pass daily self-screening for COVID-19 and agree to the terms of our Commitment to Confidentiality detailed below. Please read the following section in full before volunteering. This document is presented to all volunteers when they sign up for their first volunteer spot on SignUp.com.


Daily Testing

As of Dec. 20, 2021, all volunteers are asked to complete a rapid test on the day of any volunteer activity. Volunteers are welcome to use a rapid test at home before arriving on-site, or supplies will be available for testing on arrival. If you are unable to complete a test at home in advance, we ask that you arrive 15 minutes early for your spot.


Proof of Vaccination

As of Nov. 1, 2021, all volunteers are required to show proof of full vaccination for COVID-19 to the site lead when checking in for their volunteer spot. Anyone who is not eligible to be vaccinated may not volunteer for this program.


COVID-19 Daily Self-Screening

  1. Have you been experiencing any symptoms of COVID-19?

  • cough (new or worsening)

  • fever (chills, sweats)

  • headache

  • runny nose or nasal congestion

  • sore throat

  • shortness of breath or difficulty breathing

  1. Have you been in close contact with anyone who has tested positive for COVID-19?

  2. Have you been to any exposure sites identified by Public Health?

  3. Are you currently required to self-isolate by Public Health for any reason (travel, exposure notification, etc.)?

If you answer yes to any of the above questions, do not sign up for a shift. If your answers change between now and the time of your shift, you must cancel your shift.


Commitment to Confidentiality

While working or volunteering with the COVID-19 Pop-up Testing Events I acknowledge that I will be given access to personal information about individuals being tested that is deemed sensitive and/or confidential.

I agree that:

  1. I shall not share this information, material or documents (information) with persons within or outside of the testing events who are not authorized to have this information.

  2. I shall not publish such information.

  3. I shall not communicate such information without authority.

  4. I shall not use or disclose any such information for other than authorized official purposes.

  5. I shall not remove any such information from the premises without permission.

  6. Should I receive any such information, I will accept full responsibility to ensure the confidentiality and safe keeping of this information.

  7. I shall take every reasonable step to prevent unauthorized parties from examining and/or copying any such information.

I understand that these rules apply both during and after my participation in any of the COVID-19 Pop-up Testing Event(s) and that any infringement by me of these rules may be grounds for legal action.


If you are unable to accept the terms of this agreement, or are unable to pass the self-screening questions above, please cancel your volunteer spot using the link in your confirmation email.
